What Does this CT Scan Result Mean?

after CT-BRAIN(Plain) examination clinical profile h/o right side paralysis 4 years back now c/o convulsion. findings: a wedged shaped hypodensity is seen in the right anterior basifrontal region adjoining the cortex. mass effect is noted in the form of effacement of the sulci. a large area of gliosis with cystic encephalomalecia and ex-vacuo-dilatation of the left lateral ventricle. similar small area of gliosis is seenin the right fronto-parietal region. corresponding atrophy of the left mid brain is noted. posterior fossa structures appears unremarkable visualized calvarium appears unremarkable moderate mucosal thickening is seen in left maxillary sinus no evidence of bleed or intracranial space occupying lesion! what should be next care, treatment to be taken kindly inform! what is the risk factor bcz age of the patient is 55 with diabetes and heart problem from last 20 years! 2 bypass and 1 angiography operation with 4 years back paralysis attacak

I will try to explain the scan findings:
The wedge-shaped area on the right frontal region can represent an infarction which is a type of stroke where blood supply is cut off or decreased so much so that the cells in that region has stopped functioning or possibly damaged. This may lead to paralysis or other symptoms. However, judging from the symptoms of convulsion, there is a possibility of a tumor or infection in that location because it is uncommon for stroke to present with convulsion.
Gliosis and other findings are results of the stroke that happened four years back. In an unlikely scenario, these also can cause convulsion.
Mucosal thickening in maxillary sinus is due to sinusitis and most often benign.
I suggest that you should get an MRI of the brain, preferably with contrast, to evaluate the right frontal region lesion and consult a neurologist.
